Responsibilities
As a Home Health Physical Therapist, your responsibilities will include:
- Providing one-on-one physical therapy to patients in their homes
- Conducting assessments and developing individualized care plans
- Educating patients on their plan of care, exercises, and self-care techniques
Requirements
To be considered for this role, you must have:
- A graduate degree from an accredited Physical Therapist program and active PT license in Colorado
- At least one year of experience in physical therapy, preferably in home healthcare
- BLS/CPR certification and knowledge of state and federal home health regulations
- Strong clinical judgment, interpersonal skills, dexterity, and compassion
