About the Role:

• Lead a team of home health clinicians to ensure safe, effective, and efficient clinical practice.
• Assure ongoing interdisciplinary assessment of patients and promote patient progress toward mutually established goals.
• Supervise paraprofessional and licensed professional team members to foster a supportive and collaborative work environment.
• Use clinical decision-making to efficiently manage an assigned patient population and drive exceptional outcomes.

Requirements:

• A graduate of an accredited Physical Therapist Program and active PT license in Colorado.
• A minimum of three years of physical therapy experience in home healthcare, preferably with management experience.
• Two years of OASIS experience is preferred, along with CPR Certification and computer proficiency.
• Ability to work independently, provide effective communication to patients/families, team members, and other healthcare professionals.
