Your Role and Responsibilities
As a Home Health Physical Therapy Assistant, you will play a vital role in delivering exceptional patient care and outcomes. Your primary duties will include:
- Providing personalized, hands-on care to patients in their homes under the supervision of a licensed physical therapist.
- Assisting with treatments, documenting patient progress, and collaborating with caregivers and other healthcare professionals to ensure seamless care delivery.
- Participating in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
