Job Responsibilities
- Provide home-based physical therapy services to patients who require assistance with mobility, strength, and functionality due to illness or injury.
- Develop and implement customized therapy plans in collaboration with patients, family members, and healthcare professionals.
- Assess patient needs, document progress, and modify care plans as needed to ensure optimal outcomes.
Qualifications and Requirements
- Graduate of an accredited Physical Therapist Program and active PT license in Colorado.
- Minimum one year of experience in physical therapy, preferably in home healthcare.
- BLS/CPR certification (demonstration course required).
- Knowledge of state and federal home health regulations.
