Interim HealthCare seeks a skilled Home Health Registered Nurse (RN) Case Manager to join our team. In this role, you will work closely with patients, caregivers, and healthcare teams to provide high-quality, patient-centered care.
Responsibilities
- Complete nursing initial and ongoing assessments of in-home clients.
- Develop and evaluate plans of care in partnership with patients, representatives, and caregivers.
- Participate in interdisciplinary care coordination and assessment.
- Perform patient home visits to evaluate the quality of care being provided.
Requirements
- Active RN license in Colorado.
- Minimum of one year of nursing experience, ideally in home healthcare.
- CPR Certification required.
- Pass a background check and drug test.
- Knowledge of state and federal home health regulations.
