About the Job
As a home health physical therapist, your primary responsibilities will include providing one-on-one patient care, conducting assessments, documenting progress, and modifying care plans as needed. You will work closely with a home health team, which may include an RN, LPN, CNA, HHA, OT, and SLP, to achieve patient goals.
You will assist patients with exercises and hands-on therapy to manage pain and increase mobility, educate patients on their plan of care, established goals, exercises, and self-care, and coach families on safe exercise techniques.
