Job Description

We are seeking a highly motivated and experienced Physical Therapist to fill the role of Therapy Clinical Manager - Physical Therapist (PT) at Interim HealthCare in Fountain, CO. This full-time position comes with a competitive salary range of $90,000 - $100,000 per year and offers opportunities for professional growth and development.

About the Role

In this role, you will lead a team of physical therapists and other clinicians to deliver exceptional patient care in the comfort of patients' homes. You will oversee the development and implementation of patient care plans, supervise and mentor staff members, and collaborate with interdisciplinary teams to achieve desired outcomes.

Responsibilities and Requirements

• Lead a team of physical therapists and other clinicians to provide high-quality patient care in the home setting.
• Develop and implement patient care plans in accordance with OASIS documentation and charting requirements.
• Supervise and mentor staff members to enhance their skills and knowledge, promoting a culture of continuous improvement.
• Collaborate with interdisciplinary teams to coordinate patient care and achieve desired outcomes.
• Must have a graduate degree from an accredited Physical Therapist Program and an active PT license in Colorado.
• At least three years of experience in home healthcare, preferably as a manager or supervisor.
• Proven leadership and communication skills, with ability to work independently and as part of a team.
• Familiarity with OASIS documentation and charting systems.
