GENERAL SUMMARY: The Unit Coordinator is an integral part of a multi-disciplinary team of mental health clinicians. Provides or coordinates all covered services which includes case management, outreach, crisis intervention, and counseling to address clients' individual treatment needs. Coordinates, monitors, and provides direct services for members. Ensure individuals are safe, healthy, and staying in a clean environment while in the facility. Manages daily operations of therapeutic milieu. Will be responsible for encountering services within the Electronic Health Record.
JOB RESPONSIBILITIES:
• Engage with members to coordinate services, to facilitate linkage, referral, crisis management, advocacy, and follow up with community resources for treatment needs.
• Conducts a comprehensive psychosocial assessment and makes treatment recommendations, summarizes primary presenting issues, identifies child/family strengths, and formulates a diagnosis based on this initial interview.
• Works with staff to develop interim service plans that identify the next steps taken after intake by the provider and person/family, and what issues are to be addressed for ongoing assessment.
• Provides effective crisis assessment including the following of program triage protocol, arranging for emergency psychiatric evaluation/hospitalization as indicated by protocol, provides for follow-up, normalizes situation, discusses options and handles situation calmly.
• Completes SMI Determinations for new enrolled members and existing members.
• Advocate and articulate the client's current psychiatric needs to medical and clinical staff.
• Ability to work independently within a multi-disciplinary team context.
• Ability to provide individual support for a wide range of diagnoses and behaviors.
• Monitor and organize logistical aspects of the unit including shift coverage and task delegation, meals, supplies, and facility needs.
• Arranges transportation of members to behavioral health/medical appointments as necessary.
• Attends and participates in team meetings and clinical staffing's.
• When indicated in member's service plan will provide rehabilitation services that include; living skills training, cognitive rehabilitation, and supported employment services to members in ECU.
• When indicated in member's service plan will provide support services that include; case management, personal assistance, family support, peer support, respite care, and transportation services to members in ECU.
• Document all services rendered in member's electronic health record the same day of service.
• Submit deliverable reports to the Regional Behavioral Health Authority.
• Identify services consumers should receive based on assessment of treatment, risk factors, and immediate needs.
• Involve family and significant others in the consumer's treatment and provide psycho¬ education.
• Assess and provide supports to consumers to address needs for problem-solving, wellness self-management, housing, education and vocational training, social supports, and employment.
• Act as advocate and liaison for consumer's activities to seek and gain access to treatment.
• Responsible for following any policies, procedures, and controls established by the organization, the HIPAA Privacy Officer, and/or the HIPAA Security Officer regarding access to, protection of, and the use of the PHI.
• Performs other related duties as assigned.
QUALIFICATIONS:
Education – Master’s degree in behavioral health field preferred, bachelor’s degree in a related field required.
Experience – At least two (2) years of experience working in the public behavioral health system. At least one (1) year of experience placing clients in programs, housing, and rehabilitative services. One (1) year of experience in assessments and referrals.
REGULATORY:
• Minimum 18 years of age.
• Arizona Level One Fingerprint Clearance Card (must possess upon hire and maintain throughout employment).
• CPR, First Aid & AED certification, if required (must possess upon hire and maintain throughout employment).
• Valid Arizona Driver’s License, 39-month Motor Vehicle Report, proof of vehicle registration and liability insurance that meet company insurance requirements, if required.
