Job Description : Position Summary

In this position, the Coder II is responsible for coding of medical services, posting of charges and payments, assisting patients with account questions, acting as a coding resource to others and other billing

and claim processing services.

Essential Duties and Responsibilities

Billing and Claims Processing

Posts charges to patient accounts, prepares, verifies, and sends electronic and paper claims to

third party payers, researches incomplete, incorrect, or outstanding claims and submits claims

with knowledge of all insurance plans and contractual arrangements affecting payments

Investigates and resolves claims submission problems with third party payers and reviews new

and existing third party claims processing information

Posts payments and adjustments to patient accounts, verifies and balances cash and receipts and

prepares regular deposits and processes refunds to payers or patients in a timely manner

Scope

1.Assigns ICD, CPT and HCPCS coding classifications based on clinical documentation and / or physician orders. Utilizes appropriate tools, resources and guidelines to determine codes and assigns first listed diagnosis and secondary diagnoses. Obtains clarification from physicians, clinical departments and others on documentation questions, as needed. Performs coding at an advanced level of coding complexity. Codes complex diagnoses, CPT and assigns modifiers for multiple facilities. Codes combinations of outpatient hospital charts.

Maintains assigned work queues within defined processing timeframe and meets or exceeds productivity standards.

Verifies data abstracted and entered from the EMR. Ensures integrity of the database for internal and external data reporting.

Responds in a timely manner to inquires from Revenue Services related to use of codes and modifiers within the billing process to assure accuracy and avoid delays in the billing process.

Adheres to all compliance guidelines, both internal and external. Participates in continuing education programs to maintain an understanding of anatomy, physiology, medical terminology, disease processes and surgical techniques to support the effective application of coding guidelines and maintain credentials.

Minimum Qualifications

Education, Experience and Licensure / Certification Requirements Requirements are representative of the minimum level of knowledge, skills and / or abilities necessary to perform the essential functions of the position 1. High School Diploma or equivalent 2. Minimum three years of business office experience in a healthcare setting including billing and claims processing, insurance and CPT and ICD-10 coding 3. Minimum one year experience in a coding position for physician services with demonstrated knowledge of CPT / HCPCS Procedural Coding and ICD10 diagnostic coding 4. Current certification as CPC (Certified Procedural Coder) 5. Ability to communicate effectively and diplomatically within a multi-functional team 6. Strong organizational skills and attention to detail 7. Ability to successfully function in a fast paced, service oriented environment 8. Experience in understanding and usage of computers, including the Microsoft Office Suite, as well as the ability to learn applications relevant to the position.

Preferred

Associate Degree in Health Information or in a Healthcare related field

Minimum of three years coding experience in a hospital or acute care setting is required, using both ICD and CPT coding preferred

Physical Requirements :

Hearing / listening, lifting, manual dexterity, pulling / pushing, seeing, sitting, speaking, squatting / kneeling, standing, and walking.
