Floating Shifts:The Emergency Department Social Work Care Manager will work on a 24-hour float schedule, supporting our emergency departments at various Intermountain Health hospitals. Shift times may vary and are available 7 days a week in 6- and 12-hour increments during early morning, day, evening, and overnight hours.

Key Responsibilities:
- Care Coordination: Develop and implement comprehensive care plans to address patients' diverse needs, ensuring timely and effective service delivery.
- Education and Support: Provide patients and caregivers with tailored education and support to empower them in managing their healthcare requirements.
- Clinical Assessment and Intervention: Conduct thorough clinical assessments using standardized screening criteria and diagnose various mental, emotional, and behavioral disorders according to DSM 5 guidelines. Design and implement interventions to address patient needs, promoting optimal health outcomes.
Requirements:
- A Master's Degree in Social Work from an accredited institution
- Current State Licensure as a Licensed Certified Social Worker (CSW) or Clinical Social Worker (LCSW)
- Proficiency in basic computer skills and knowledge of word processing and spreadsheet software
