Job Description
The Clinical Social Worker will utilize clinical expertise to perform psychosocial assessments, develop and implement plans of care, and provide individual and family treatment as indicated. This role requires strong communication and interpersonal skills, as well as the ability to work collaboratively with patients, families, healthcare providers, and other involved parties.
Responsibilities
- Patient Assessment: Conduct thorough assessments of patients' medical history, mental health history, and current presentation to identify their needs and develop effective treatment plans.
- Treatment Planning: Develop and implement plans of care in collaboration with patients, families, healthcare providers, and other members of the healthcare team.
- Counseling and Therapy: Provide individual and family counseling and therapy as indicated, utilizing evidence-based interventions and techniques.
- Referrals and Resources: Identify and refer patients to other services and resources that can improve their well-being, such as community services, treatment programs, support groups, and psychiatrists.
