This position requires a deep understanding of clinical expertise to perform comprehensive psychosocial assessments, develop and implement effective plans of care in collaboration with the care team.
">Key Responsibilities:

- Diagnostics and treatment of various mental, emotional, and behavioral disorders ">
- Modifying care plans to reflect changes in patient status ">
- Referring patients to other services to improve their well-being ">
- Collaboration with the multi-disciplinary team to develop a discharge plan ">
- Organizing and prioritizing daily work by assessing new, current, and discharging patient needs ">
- Performing behavioral health consultation for patients and caregivers ">
- Conducting psychosocial assessments ">
- Providing supportive, crisis bereavement and other behavioral health interventions as necessary ">
Requirements:

- Masters Degree in Social Work or related field from an accredited institution ">
- Current State Licensure as LCSW or LMFT, or LPC ">
- Excellent written and verbal communication skills ">
- Computer Skills, i.e., email, word processing ">
- BLS certification or ability to obtain within 90 days of employment ">
