Key Responsibilities :
- Perform comprehensive assessments of patients' physical, social, and psychological needs.
- Develop and implement individualized care plans in collaboration with healthcare teams and patients.
- Administer medications and treatment while prioritizing and organizing patient care delivery.
- Evaluate patient responses to interventions and make necessary adjustments to care plans.
