We are seeking a skilled and compassionate Physical Therapist to join our dedicated team. As an Outpatient Physical Therapist, you will evaluate, diagnose, and treat patients with physical impairments, promoting healing and mobility through personalized therapy plans. Your expertise will help patients achieve optimal recovery, regain independence, and reintegrate into their communities.

Key Responsibilities:

• Competent Services: Developing treatment plans that address patients' specific needs, including exercise regimens, adaptive equipment recommendations, and pain management strategies. Performing and documenting initial and ongoing assessments of patients' conditions.
• Documentation and Monitoring: Tracking patients' progress, making adjustments to treatment plans as needed, and providing regular updates to patients, families, and healthcare providers. Collaborating with multidisciplinary teams to ensure seamless communication and coordinated care.
• Communication: Effectively communicating with patients, families, physicians, and healthcare providers about patient needs and goals, fostering a supportive and collaborative environment.
• Environment Maintenance: Maintaining a clean and welcoming work environment, adhering to health department codes and facility standards.
• Research and Development: Participating in research initiatives and staying up-to-date with the latest advancements in physical therapy practices and technologies.
