Job Summary
The Behavioral Health Care Coordinator functions as a member of the collaborative care team that includes the patient, primary care medical / dental, behavioral health, psychiatric providers; and other medical staff. This is a non-clinical position that does not provide therapy or counseling. This position is responsible for providing integrated behavioral health care within a primary care setting, which includes planning, organizing, and coordinating behavioral health services in conjunction with other members of the patient’s care team and identified external providers who impact patient care, including support with behavioral health services care gap metrics.
Education - Bachelor’s degree in a mental health field (social work, psychology, counseling, or related field).
Experience - One (1) year experience working with individuals with co-occurring mental health and / or substance use disorders; individuals with a disability; individuals of a vulnerable population (elderly, mothers, children) and / or other high risk groups preferred.
Other Requirement(s) - Suicide assessment, treatment, and management training required by WA State Department of Health within sixty (60) days of hire.
