Summary
Program Director provides administrative management according to the agency policies and procedures. Worh effectively as a team memb r in a dynamic environment and promotes teamwork within and across the organization by maintaining po itive r lationsllips. Program Director reports directly to Clinical Dir:ectOl' and works with clinical staff and other team members.
Responsibilities:
Essential Functions:
- Provides direction and leadership to office staff and clinical team members in collaboration with the clinical director.
- Assures compliance with and ensures timely follow up on daily tasks.
- Collaborates with clinical director and administrative staff to ensure productivity targets and patients satisfaction.
- Collaborates with all health care providers to ensure the timesheets and accuracy of the work flow (MD orders, Plan of Care and others).
- Collaborates to receive referrals and assign appropriate clinicians for timely patient evaluation by signing off after authorization and plotting start of care (SOC) visits.
- Collaborates with other team members to coordinate determination of patient home health benefits and ongoing insurance approvals.
- Ensures adequate communication flow within the office and with all clinicians.
- Coordinates communication of care with all disciplines, physicians, durable medical equipment providers, caregivers/family members, transferring facilities, and any other applicable healthcare providers.
- Reviews clinician visit notes to ensure timely, complete, appropriate, and accurate submission of all documentation by field staff. Takes necessary action to correct adverse findings and communicates trending to clinical director.
- Processes new orders and updates visit frequency, as appropriate.
- Participates in receiving report from field clinicians prior to scheduled days off on patient status and ongoing needs.
- Participates in on-call rotation.
- Maintains strictest confidentiality of medical/company information.
- Other duties might be assigned according to agency's needs.
- Additional COVID-19 Requirements: Must complete COVID-19 trainings and comply with COVID-19 policies and practices, including those related to temperature screenings, symptom reporting, travel restrictions, social distancing, aod face coverings (all accommodation requests must be submitted to the HR Director).
Nonessential Job Functions:
- Participles in alltrainings.
Oualificatioru:
- Must possess positive, enthusiastic attitude, good people skills, and professional appearance, as well as excellent telephone skills.
- Must be able to stoop and bend.
- Must be able to lift and carry effectively so as to be able to perform the above listed functions.
- Must be able to carry equipment weighing up to 10 pounds upstairs.
Working Conditions/Physical Demands:
Ability to access all areas of the facility through the workday. Ability to lift up to 50 pounds occasionally, and up to 25 pounds frequently, and up to 15 pounds constantly. Requires constant hand grasp and finger dexterity; frequent sitting, standing, walking, and repetitive movements, occasional bending, reaching forward and overhead; squatting, kneeling. The person in this position needs to occasionally move about inside the office to access file cabinets, office machinery, etc.
Constantly operates a computer and other office productivity machinery, such as a calculator, copy machine, fax machine, and computer printer. Requires clarity of vision at 20 inches or less. Ability
to communicate verbally and in writing with an excellent comprehension of the English Language. Work is generally preformed in an indoor, well-lighted, well-ventilated, heated, and air conditioned environment. Must be able to read 12 point or larger type. Must be able to hear adequately with no more than an amplifier on the phone and to speak in a manner understood by most people.
