International Quality Homecare: is seeking a Full-Time RN Case Manager for our Mankato, MN branch office.
Basic Qualifications:
The RN Case Manager position requires a minimum of an Associate degree in Nursing (RN) with at least 2 years of experience in home health or Nursing home. The Nurse must be licensed to work in the state of practice. The Nurse must have a valid driver’s license and a good driving record.
Other Qualifications:
The position requires a clear understanding of nursing in-home care settings. The ability to assess the needs of the clients and work with individuals within and outside the company to meet the needs of the client is required. The position requires a demonstration of strong analytical, problem-solving, communication, and leadership skills. Computer proficiency and the ability to learn different computer programs are highly required. Supervisory experience is preferred but not required.
Responsibilities:
Prepares OASIS and other medical documents. Provides nursing care to the clients.
Field Supervision of Field Staff communicates concerns regarding field staff to Operations Coordinator for performance management and any necessary disciplinary action.
Resolves Nursing and Administrative issues.
Works with Caseworkers, Counties, Hospitals, and other parties involved in the case.
Represents the interest of the company, client, and field nurses.
