International Quality Homecare Corporation: is seeking a Full- time or Per diem RN Case Manager for our Rochester, MN branch office.
- The Per Diem position is $40.00 - $50.00 per visit, travel time $10.85 per hour, and .40 per mile between clients.
Basic Qualifications:
The RN Case Manager position requires a minimum of an Associate degree in Nursing (RN). The Nurse must be licensed to work in the state of practice. The Nurse is expected to have a valid driver’s license and a good driving record.
Other Qualifications:
The ability to assess the needs of the clients and work with individuals within and outside the company to meet the needs of the client is required. The position requires a demonstration of strong analytical, problem-solving, communication, and leadership skills. Computer proficiency and the ability to learn different computer programs are highly required. Supervisory experience is preferred but not required.
Responsibilities:
Prepares OASIS and other medical documents. Provides nursing care to the clients.
Field Supervision of Field Staff communicates concerns regarding field staff to the Operations Coordinator for performance management and any necessary disciplinary action.
Resolves Nursing and Administrative issues.
Works with Caseworkers, Counties, Hospitals, and other parties that are involved in the case.
Represents the interest of the company, client, and field nurses.
