The Care Manager is a Registered Nurse embedded in a local nephrology practice. The Care Manager is responsible for promoting quality care for our kidney members by providing guidance, support, education, and other care coordination functions as defined by the pertinent program. You will implement the pertinent program as assigned while acting as the subject matter expert and resource for members, colleagues, dialysis staff, physicians, and other healthcare professionals.
This is an onsite, location-based position. Additional details on the specific location will be provided in the interview process. This position is eligible for a one time sign-on bonus of $10,000.00.
The work you will do:
- Collaborate with clinical and non-clinical staff and other members of the patient’s health care team to develop individualized, holistic and kidney care interventions that meet the member’s needs.
- Communicate and coordinate with the patient and appropriate members of the patient’s health care team to develop, execute, and review results of care coordination efforts.
- Utilize motivational interviewing techniques to influence members to engage in their own care.
- Collaborate with providers and members to ensure an optimal and successful transition from chronic kidney disease to renal replacement therapy of choice.
- Partner closely with providers and dialysis clinics to identify and mitigate hospitalization and mortality risk for members.
- Participate in quality improvement activities and project-based work including practice, clinic, and community-based education initiatives.
- Evaluate program processes and make recommendations to management that will improve the effectiveness and efficiency of the program.
The skills and qualifications you need:
- Registered Nurse (RN) required with active unrestricted Maine licensure, or a clear and active Nurse Licensure Compact (NLC).
- Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred.
- Minimum 3 years' experience in clinical nursing, preferably within renal care, ambulatory care, or complex case management.
- Ability to develop trusted patient and caregiver relationships to drive engagement and outcomes.
- Must have knowledge of teaching-learning process, principles, and methods of adult education.
- Proven understanding of primary care as well as kidney, cardiovascular and diabetes disease processes and their basic management practices.
- Understanding of dialysis modalities and transplant evaluation process is preferred.
- Excellent and professional communication skills.
- Proficiency with navigating electronic medical records (EMR) and adeptness with care documentation.
- Strong computer skills, proficiency in MS Word and Outlook.

Pay: $66,260.25 - $88,347.00 per year
