Summary:
We are seeking an RN experienced in leading a phone-based care coordination program focused on Remote Patient Monitoring (RPM), disease management, care coordination, and social determinants of health alignment. The Manager will be responsible for training and managing staff, taking patient escalations, managing patient outcomes, and responsible for driving population health performance metrics. This role requires the ability to build structure, create workflows, and hold team accountable to performance, as well as strong leadership skills and operational prowess.
Key Responsibilities:
- Lead and manage the IntraCare population health care management program, ensuring alignment with organizational goals.
- Develop and implement strategies to enhance patient engagement, improve outcomes, and reduce healthcare costs.
- Monitor program performance metrics, identifying areas for improvement.
- Oversee the training and development of the care management team.
- Ensure HIPAA guidelines are being followed for care management program
- Manage the daily operations of the care management program, including patient specific disease management, device monitoring, discharge follow-up outreach, and social resource alignment.
- Analyze program data to track patient outcomes and identify opportunities for improvement.
- Maintain relationships between the care management team and provider practices
Qualifications:
- Current RN license in the states of Arizona and Texas
- Bachelor’s degree in healthcare administration, nursing, or related field
- Minimum of 3 years of experience managing a chronic care management and/or remote patient monitoring program
- Excellent communication and leadership skills.
- Experience with healthcare technology and EHR systems
