What are the responsibilities and job description for the Targeted Case Manager position at Intrust Healthcare?
Intrust keeps GROWING and we are in need of a team-oriented Case Manager to service Pulaski residents. This position starts as PT but can quickly grow to FT. This is an in-person position in our Somerset office with the potential for GROWTH. We offer in-house required training for CORE, SMI and SED and is FREE to you as an employee.
Case Managers work to assist clients and families with alleviating problem behaviors by:
- -Linking your clients with services that are tied directly to the client's diagnosis.
- -Providing assistance in coordination of treatment through monitoring services.
- -Linking client and family to needed community resources and referrals.
- -Helping prepare client and family to function independently after services are complete.
Education Requirements:
Bachelors degree in Human Services related field plus one year of post graduate experience. OR- A Master's Degree in a Human Services related field (there is no experience requirement if you have a Master's)

Any Estimated Yearly Salary amounts are based on a 40 hour work week with income potential above 50k per year.
