We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care in various settings, including primary care, outpatient clinics, and nursing homes. As an LPN, you will play a crucial role in supporting patients' health and well-being through effective communication and clinical skills.
Duties
- Conduct routine assessments of patients, including taking vital signs and recording medical histories.
- Maintain accurate medical records and documentation for each patient encounter.
- Assist in case management by coordinating patient care plans and follow-up appointments.
- Provide direct patient care in outpatient settings, ensuring comfort and safety.
- Support internal medicine practices by preparing patients for examinations and procedures.
- Perform phlebotomy as needed for laboratory testing.
- Participate in triage processes to prioritize patient needs effectively.
- Collaborate with healthcare professionals to deliver comprehensive care to patients in nursing homes or during surgical procedures (OR).
Experience
Candidates should possess a valid LPN license and demonstrate experience in primary care or related fields. Familiarity with case management, outpatient services, internal medicine, and triage is highly desirable. Previous experience working in nursing homes or surgical environments will be considered an asset. Strong communication skills and the ability to work collaboratively within a healthcare team are essential for success in this role.
