Job Summary
This position works closely with the inpatient multidisciplinary care team to provide proactive, integrated social services care management that support the biopsychosocial needs of patients from birth to end of life.
Key Responsibilities:
- Work directly with patients and their families to provide support, plan for discharge from the hospital, identify barriers to discharge, and assist with financial and housing needs.
- Collaborate with state and federal agencies to provide information for placement needs, identify custodial issues, and refer patients in crisis to behavioral health.
- Provide supportive counseling, encouragement, coaching, and linkage with essential community services to support each individual's unique healthcare needs.
Requirements:
- Minimum 2 years of experience in a hospital environment in social work.
- Knowledge of case management principles, customer service, and recovery skills.
- Ability to maintain strict confidentiality of patient care information, work independently in a fast-paced environment, and provide advocacy and outreach.
Working Relationships:
The Social Worker will establish and maintain up-to-date knowledge and effective working relationships with local community resources, and develop and maintain a trusting and partnering relationship with assigned patients to ensure successful care and recovery.
