Your Role:
Perform in-home assessments & develop personalized care plans
Provide skilled nursing care
Educate patients and families on health management
Coordinate with interdisciplinary teams to ensure the best patient outcomes
Requirements:
✔Active RN License
✔ Home health experience (preferred, but not required)
✔ Passion for patient-centered care
✔ Valid driver’s license & reliable transportation
