What are the responsibilities and job description for the Case Manager position at IOCP-Interfaith Outreach & Community Partners?
IOCP is a nonprofit that provides services in food, case management, emergency financial assistance, employment services, and childcare resources to those residing in our service area. We are looking for two Case Managers to play a key role in delivering services to clients where they are at: virtually, at IOCP’s main offices, or at one of our neighborhood program sites.
Case Managers work 1:1 with IOCP clients to assess needs, connect to resources, develop and implement plans to work toward increased stability and the achievement of individual goals. This position supports client movement toward increased stability through general case management and extended rental assistance. What You Will Do
- General case management including initial client program enrollment, needs assessment, crisis response, goal planning, and internal/external program referral
- Support clients to develop and implement a plan to achieve stability goals; provide connections to external and internal resources, access to financial assistance, information and referral, advocacy, counseling and problem-solving assistance in support of housing retention and stability goals
- Extended rent – request and review required documentation from clients monthly to maintain ongoing program participation

Requirements:What You Will Bring
- BSW and 5 years of experience in a social services setting
- Two years of experience providing case management services to individuals and families
- Thorough understanding of the case management process
- Ability to creatively problem-solve and complete complex tasks
- Strong computer skills
- Excellent organization and time management skills
- Ability to maintain professional boundaries
- Strong interpersonal, verbal and written communication skills
- Ability to work effectively in a diverse environment and within a team
