The Visiting Nurse Association is growing and adding another Speech Therapist to work our team. Our schedule is primarily M-F between the hours of 8 AM and 5 PM. We can offer this new team member a flexible part time position (with benefits) or a PRN role with a few hours per week (without benefits). Our Speech Therapists maintain a small caseload, with visit frequencies based on patient needs.
Our ideal candidate will enjoy finding creative solutions to individualized client needs, while being engaged and immersed in building relationships with clients. This position is responsible for self scheduling a small case load, developing treatment plans that addressing solutions to living environment barriers, and assisting each client to remain comfortable, safe and independent?
We work as a professional interdisciplinary team to provide Home Making, Aide, Nursing, Physical Therapy, Occupational Therapy and Speech Therapy services to clients, so they can remain independent and enjoy life in their home.

The Essential Duties :
1. Provides comprehensive Speech Therapy services to individuals and families, assessing the patient in the home setting and advises / consults with the patient and family, making recommendations and / or initiating changes to facilitate communication and patient functioning within the home.
2. Accepts primary care responsibilities for Speech Therapy and coordinates services of all disciplines as needed.
3. Plans, coordinates and provides services to special groups as assigned.
4. Complies with agency standards on visit productivity.
5. Complies with agency policies on frequency of home visits to meet individual and family needs.
6. Complies with agency policies regarding the maintenance and preparation of records and reports.
7. Provides patient medical, geographical, financial and statistical information as needed by the agency.
