Iowa City Hospice is growing! We are excited to add one more Nurse Case Manager to our team. Working at Iowa City Hospice means you get to develop many new nursing skills and add depth to your career while enjoying the one-on-one relationships you build with the people you serve. Our nurses work Monday through Friday between 8 AM-5 PM. They do share some participation in holiday rotations and occasional night / weekend call shifts. Our Nurses have a passionate understanding for the hospice philosophy. They combine their acute / crisis symptom management education with interpersonal communication skills to provide care for patients and families in physical and emotional crisis situations. We offer reasonable case loads. You will work autonomously within an interdisciplinary team of professionals. Our nurses :

  • Assess and evaluate patient needs and develop a comprehensive care plan
  • Coordinate and manage patient care with interdisciplinary team members
  • Provide education and support to patients and their families
  • Administer medications and treatments as prescribed by the physician
  • Monitor and document patient progress and make necessary adjustments to the care plan
  • Ensure compliance with all regulatory requirements and standards of care
  • Participate in on-call rotation as needed

  • Working with a wide range patient needs based on a variety of end stage illness.
  • Challenges of working to the full scope and capacity of practice using complex clinical skills and displaying a high level of confidence to complete professional assessments and monitor new or worsening symptoms.
  • Identifying and coordinating patient / family care with other members of the Interdisciplinary Team and health care community as needed.
  • Communicating with the primary physician regarding patient change in status and pain and symptom control.
  • Teaching and counseling patients / families on care giving techniques.
  • Working closely with all members of the interdisciplinary team – Physician, Pharmacist, ARNP, Hospice Aides, Social Workers, Music Therapists, Spiritual Counselors, Volunteers, and Bereavement specialists, to identify and coordinate a holistic approach to the individual care needs of the patient.
  • Facilitating admission of hospice patient / family when necessary.
  • Documenting observations, treatments, and other pertinent information into an IPad electronic medical records system.
  • Participating with the entire organization to promote confidentiality, quality assessment, and continual performance improvement.

  • 19 Days (152 hours) of accrued vacation in your first year (available to use as you accrue!), 24 days (192 hours) after three years and 29 days (232 hours) at your seventh year of employment.
  • 12 days (96 hours) sick days accrued per year (available to use as you accrue!

  • Strong communication and interpersonal skills.
  • Ability to communicate in English verbally and in writing

  • Proficiency in medical terminology and patient care documentation.
  • Qualifications :

  • Previous experience in hospice, home health, hospital or long term care and a sincere desire to help others
  • Current Iowa Registered Nurse License;BSN preferred
  • Available to work Monday - Friday, includes some evening & weekend call & Holiday rotations
  • Iowa driver’s license
  • US work authorization
  • BLS Certification
  • Mandatory Reporter Training
