Responsibilities:
- Deliver physical therapy treatments according to physician orders and the state Physical Therapy Practice Act.
- Evaluate patients’ therapeutic needs and develop personalized care plans, adjusting as needed.
- Educate patients and caregivers on the use and care of therapeutic equipment.
- Report patient progress and any changes in condition to the supervising physician.
- Participate in discharge planning and submit clinical progress reports.
- Supervise physical therapy assistants and collaborate with other healthcare team members.
Qualifications
- Degree in Physical Therapy from an accredited program (Bachelor’s or Master’s).
- Licensed to practice as a Physical Therapist in the state.
- Minimum of two years of experience in physical therapy; community/home health experience preferred.
- Strong communication, organizational, and problem-solving skills.
- CPR certification and a valid driver’s license with reliable transportation.
