Title: Home Health Intake Coordinator
Direct Report: Clinical Manager
Job Summary:
- We are seeking a detail-oriented and compassionate Home Health Intake Coordinator to join our team. As a Home Health Intake Coordinator, you will play a crucial role in ensuring the smooth and efficient admission of patients into our home health care services. You will be responsible for managing the intake process, assessing patient needs, obtaining necessary documentation, and coordinating with various stakeholders to ensure a seamless transition into our care.
Duties and Responsibilities:
Intake Coordination:
- Receive and review incoming referrals for home health services
- Work closely with healthcare professionals, patients, and their families to gather necessary information for intake assessment
Documentation and Record Keeping:
- Accurately enter and maintain detailed information in the electronic health record system
- Ensure that all required documents and forms are completed accurately and in a timely manner
- Deliver messages and faxes throughout the day to all internal and external departments
- Upload and maintain patient wound assessment documentation
- Follow up with nursing team and requesting wound documentation
- Communicate and follow up with out of Hospital patient care
- Creating and tracking non-admission reports
Patient Medical Supply Ordering:
- Supply ordering for all drop ship patients every 30 days or as needed
- Supply ordering for nursing team, once approved from clinical manager
- Follow up with patient supply ordering to ensure delivery
Additional Job Duties:
- Answer main phone line when receptionist is unavailable
- Other job duties as needed
Job Qualification and Experience:
- High school diploma or equivalent; some college coursework in healthcare administration or related field is preferred
- Medical Assistant Certification preferred.
- Proven experience in a healthcare, preferably in a home health or hospice setting
- Strong understanding of medical terminology, healthcare regulations, and insurance verification processes
- Excellent communication and interpersonal skills to effectively interact with patients, families, healthcare professionals, and team members
- Proficiency in using electronic medical records (EMR) systems and Microsoft Office Suite
- Detail-oriented with exceptional organizational and multitasking abilities
- Compassionate, patient-focused, and able to maintain professionalism in challenging situations
Working Conditions and Physical Effort: Works in a routine office environment. The occasional need to lift, pull, carry and push items up to twenty-five (25) pounds. Frequent need to stoop, kneel and reach while accessing files. Requires working under some stressful conditions to meet deadlines. Requires typing and computer proficiency.
