This role involves working closely with clinical staff and outside specialists to deliver high-quality patient care.
Key Responsibilities:
- Implement evidence-based disease management protocols and key clinical pathways to improve clinical performance. Participate in Collaboratives and healthcare programs approved by Senior Management. Ensure compliance with clinical measures.
- Coordinate referrals, including social services, for patients requiring additional support.
- Participate in staff outreach and off-site healthcare programs that require Family Medicine services.
- Deliver care services that meet the needs of diverse patient populations.
- Provide coverage at other CFHC centers as needed.
- Offer off-hours and Saturday clinic coverage as assigned by the Chief Medical Officer.
- Collaborate effectively with the clinical team and support colleagues.
Minimal Qualifications:
- Education: Residency-trained physician.
- Board certification or eligibility in Family Practice.
- Florida medical license.
- DEA certificate.
- Malpractice insurance.
- Medical staff privileges at an appropriate hospital.
- BLS certification.
