What are the responsibilities and job description for the UNIT COORDINATOR - SKILLED NURSING UNIT position at Iredell Health System?
Description The Unit Coordinator assists with the coordination and the discharge planning for SNF patients and arranges services needed upon discharge from the SNF. This position will also work as a administrative assistant to the skilled unit and Director of Nursing. The Unit coordinator has full access to patient health information. Requirements High School diploma or equivalent required Knowledge of Medicare, Medicaid, and SNF state/federal regulations preferred. Certification in Basic Life Support (BLS), through the American Heart Association and American Red Cross Must possess full range of body motion to pass basic FIT test for position to include walking, kneeling, standing, pushing, pulling, bending, stooping, reaching and sitting for extended periods of time. Must be able to lift and carry up to 30 pounds.
