Description
The Program Manager provides a high-quality Diabetes program for participants by supervising staff and directing all facets of the program. Has full access to patient health information. Provides leadership, education and self-management training materials to program participants, Cardiac and Pulmonary Rehab patients, community members, and corporate wellness participants as needed to enhance the understanding and management of Diabetes.
Requirements
- Associate or bachelor's degree from an accredited institution related skill required
- Current Registered Nurse licensure in N.C. or Registered Dietitian to practice in NC
- Strong background in diabetes education
- Prior managerial experience preferred
- Certified Diabetes Educator with current certification highly preferred.
- Completes a minimum of 15 hours of continuing education in diabetes, education principles, and behavioral strategies every year preferred by the ADA
Essential Physical Requirements
- Must possess full range of body motion to pass basic FIT test for position to include walking, kneeling, standing, pushing, pulling, bending, stooping, reaching and sitting for extended periods of time.
- Must be able to lift and carry up to 30 pounds
Job-Specific Standards
The level of performance must be measured by Customer Feedback, Documentation and Observation.
- Able to receive referral information and schedule an appointment for the participant.
- Obtains medical history and clinical needs assessment of assigned participant.
- Delivers appropriate diabetes management education and training based on results of the clinical needs assessment.
- Leads group classes and tailors' education to individual needs as appropriate, with adherence to all HIPAA guidelines.
- Delivers education and training to participant and significant others, in the outpatient setting, community and corporate environments as needed.
- Documents appropriate information related to Diabetes training, group classes and patient progress in the medical record.
- Keeps patient information confidential.
- Provides correspondence to the health care provider regarding the patient’s training.
- Provides follow up services to participants that have completed the training.
- Assess the need and/or patient request to develop weight management education and classes and promote to enhance patient outcomes.
- Assess the lifestyle and eating habits of individual patients, including those with a diagnosis of a chronic condition. Determine appropriate dietary restrictions, if applicable and makes recommendations for optimal clinical outcomes and wellbeing.
- Participates and leads in community education programs, serves as a content expert for marketing and delivers presentations as requested to promote awareness and education of diabetes.
- Prepares and submits recommendations for the annual program budget and works conscientiously to operate within budget parameters throughout the fiscal year, with assistance from the Director of Wellness.
- Chairs the Diabetes Oversight Committee and acts as liaison among program staff, advisory committee and hospital administration.
- Oversees the process and progress of maintaining ADA Recognized status including: planning, reviewing program annually, evaluation of the program effectiveness and CQI.
- Thoroughly understands the current Joint Commission standards and strives for excellence in meeting the standards.
- Ensures that patient training manuals for the Diabetes Center for Learning are updated and maintained as required to reflect evidence-based practice in Diabetes education, management and care.
- Maintains adequate staff: interviews, hires, orients, counsels, disciplines, and terminates employees with the assistance of the Director of Wellness. Ensures continuing education requirements are met for all employees.
- Completes staff performance evaluation in a timely manner.
- Participates in developing and keeping staff current on policies and procedures related to personal protective devices and Standard Precautions practices in order to comply with OSHA standards.
- Oversees inventory of educational materials.
- Dedicated to the growth of the diabetes outreach and Medical Nutrition Therapy (MNT) through maintaining a strong referral base.
- Actively participate and contribute to marketing plans and strategies to highlight programs, while exploring continued growth and development of programs that are consistent with the mission and strategic plans of the health system.
- Demonstrates ability to use a computer for the following functions: Word Processing, Excel, PowerPoint, E-mail, Relias, Paylocity and Cerner.
- Demonstrates the ability to use all forms of office equipment including: fax, coping machines and LCD projector.
