What are the responsibilities and job description for the Health Educator position at Iris House?
POSITION SUMMARY
The Health Educator is responsible for the coordination of health education workshops and group nutritional education classes, the review of client social determinant of health assessments, food vouchers, referrals, program eligibility documents, and the tracking and documenting clients’ group attendance and progress in the program. The Health Educator will also ensure accurate data collection and entry into the charts and outcome measurement.
ESSENTIAL POSITION FUNCTIONS
- Adheres to funder and agency requirements for the provision of Nutrition Health Education and Health Education workshops.
- Develops nutrition education and health education workshops that incorporate goals and objectives, frequency and duration of sessions, and facilitate on HIV related health and nutrition education topics.
- Work in collaboration with the Case Manager and Intake and Service Coordinator/Program Manager to ensure client services are met and documented on a monthly basis.
- Perform and review social determinants of health assessment for food insecurity and adequate food resources and makes appropriate referrals when necessary.
- Conduct receipts review of food vouchers and reconciliation of food vouchers with program participants, and documenting progress notes and in reporting database.
- Responsible for maintaining client records and works as part of the Continuous Quality Improvement (CQI) and evaluation team for program improvement.
- Maintains client records that document group attendance, participation, progress, accomplishments, and additional needs, referrals and goals.
- Monitors, in conjunction with the Senior Program Director, the appropriateness of the food resources and service provided to the client.
- Establishes connections, communicates frequently with Medical Providers/ Nurse Practitioners, and RDs, and makes referrals as necessary and appropriate.
- Review program intake/ screening form, and periodically complete rescreening form to determine reasons for client participation in the program.
- Ensure clients’ adherence to all program policies, procedures, and follows program protocols for participation, voucher distribution, food pantry, health workshops, and incentives.
- Participate and take lead in program’s community coordination activities, including monthly team meeting held by Senior Director of Health Initiatives.
- Communicates and collaborates with Funder and responds timely and appropriately to all requests from funder, Senior Program Director, and Executive Director.
- Attend and successfully complete all required training and represent the agency at various venues promoting the program and the agency.
- Perform other duties as assigned by the Program Director and Senior Director of Health Initiatives.
OTHER POSITION FUNCTIONS
- Serves as a liaison between Iris House and program providers and partners.
- Demonstrates teamwork, open communication, and has good interpersonal skills.
- Participates in in-house and community presentations, research and grant writing.
- Performs other related duties as assigned.
- This position is fully in-person, and all services are rendered at our West Side Headquarters.
POSITION REQUIREMENTS
Education: AA/AS or BA/BS in Nutrition Science, Community Health, or related field such as Health Education or Family and Consumer Sciences.
Experience: 2-3 years’ experience working in the field of HIV/AIDS or with other chronic illness; 2-3 years’ experience developing and facilitating nutrition education and/or health education.
Other Requirements: Bilingual (Spanish/English) strongly preferred; but not required. Must have excellent presentation and writing skills. Possess an understanding of community level work and the importance of collaborating with other agencies. Position requires cultural and linguistic competency with the target population.

Pay: $44,000.00 - $50,000.00 per year
