What are the responsibilities and job description for the Case Manager position at Iris Wellness Group?
Duties
- Responsible for holding clients accountable to meeting their treatment commitments; such as group attendance, individual session attendance, and the completion of scheduled drug screenings in a timely manner.
- Responsible for overseeing client activities which allows for effective and efficient operations. It involves collaboration with the Iris Wellness Group staff, interns, and volunteers as well as the administrative staff. Responsible for providing a safe and supportive environment for clients.
- Ensures clients are present on days and times they are scheduled for treatment. This includes ensuring clients are present for groups, individual sessions, and scheduled drug screens.
- Communicate client needs to the Program Director as they arise.
- Follow up with clients to ensure they are completing all assigned paperwork or activities as they are assigned by their therapist.
- Engage in crisis interventions, using de-escalation techniques and other clinical modalities or skills.
- Facilitate groups that vary based on the individual staffs scope of practice and competency. Documentation must be completed for group sessions in an efficient and timely manner.
- Collaborating with clinical staff to ensure quality client care is being delivered.
- Answering incoming calls as needed and documenting information from those conversations on the appropriate forms.
- Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered
- Facilitate multiple care aspects (case coordination, information sharing, etc)
- Arrange abeyances with social services, health and governmental agencies
- Responsible for admitting new clients to the program by completing their intake paperwork with them, orienting the client to the program rules and guidelines. Responsible for completing bag and personal searches for all new admission. Responsible for communicating new admission information to all staff via email, following the company’s procedures. and completing bag searches upon new intakes.
- Being knowledgeable on the company’s intake process and how to conduct an intake thoroughly, in a timely manner
- Applies confidentiality rules and regulations appropriately
- Demonstrates professionalism and respect in all contacts with prospective clients, new clients, current clients, and their loved ones
- Orients all new clients to the program upon admission and conducts routine Orientation Groups for all new clients
- Personal skills that assist in helping the client feel safe and comfortable entering a new environment
- Responsible for the accurate, timely and complete documentation of materials requested. Responsible for issuing reports and communications that are called for in the completion of assigned duties. Ensures for the efficient operation of the site in conjunction with other team members. Understands the organization’s CQI Plan and complies with all monitoring, documenting and reporting requirements.
EDUCATION:
- High School Diploma or GED
- Completion of Microsoft Office XP, word, and Excel programs/courses
Qualifications
- 1 years’ experience in the field of Substance Abuse and/or Mental Health
- Working knowledge of 12 step programming helpful
- Valid Driver’s license, with no incidents in the last 3 years preferred
- Organizational and administrative skills
- Able to communicate effectively (both written and oral skills)
- Ability to work independently
- Ability to work under pressure
- Familiarity with health and safety issues
Job Types: Full-time, Part-time
Pay: $13.00 - $15.00 per hour
Expected hours: 13 – 40 per week
