What are the responsibilities and job description for the Care Manager - Behavioral Health - Full Time position at IRMC?
The Case Manager coordinates the provision of direct patient care according to Nursing Care Standards. Promotes and adheres to the IRMC core values of ICARE (Integrity, Compassion, Accountability, Respect, and Excellence) and G.R.E.A.T. (greet/respond/explain/ask/thank) customer service standards.
Essential Duties and Responsibilities
In this role you will be:
- Using thorough knowledge of age-specific needs, care of mentally ill persons, Chapter 5100 of the M.H. Procedures Act, CMS and CIHQ standards, voluntary, involuntary, patient rights and confidentiality.
- Responsible for the overall provision of case management services including utilization management, discharge planning, and care management functions.
- Establishing and maintaining efficient methods of ensuring hospital admissions’ medical necessity and appropriateness.
- Collect and maintain data for readmission and patient placement.
- Responsible to verify completion of pre-admission certifications, complete concurrent reviews with insurance providers, complete appeals to denial process, maintain tracking of approved patient days on all patients within the facility.
- Attends treatment team meetings to maintain open communication with treatment team and verify documentation for continual stay is accurate.
- Exemplifying the IRMC mission, vision, and values, acting in accordance with IRMC policies and procedures to ensure quality care is provided.
- Utilizing excellent interpersonal communication skill to effectively communicate with patients, family, and members of the organization.
- Will be required to function in all units, including geropsych, adult, and adolescent.
QUALIFICATIONS
Required:
- EDUCATION:
- Graduate of an accredited nursing program.
- Bachelor’s degree in nursing or related field
- EXPERIENCE:
- 2-3 years med-surg experience and/or 1-year geriatric experience
- 1 year of behavioral health experience
- 2-3 years Case Management experience
- LICENSURE/CERTIFICATION:
- Possess current license as an RN in Pennsylvania or must pass nursing boards within 6 months of hire.
- Case Management certificate.
- BLS required or obtained within 1 month of hire.
