DUTIES AND RESPONSIBILITIES
1. Promotes the mission, vision, and values of the organization.
2. Compassion and understanding of the elderly.
3. Work as a member of the Interdisciplinary Team.
4. Initiate and complete assessments, MDS, CAAS and care plans in accordance with the MDS process as outlined in the RAI Manual and regulations.
5. Update care plans and ADL sheets as scheduled and as needed.
6. Implement each resident’s person-centered care plan through action and education.
7. Keeps current in MDS trends and new developments.
8. Sees that residents and families are involved, consulted, and empowered by the care planning process.
9. Assures implementation of each resident’s person-centered care plan through action and education
10. Facilitate all resident care conferences, assuring compliance with all State and Federal regulations.
11. Assures the care conferences are moving along, focused and on time.
12. Provide an open forum where residents, their family and their caregivers can review a resident’s problem, needs and strengths to develop the best possible plan of care.
13. Ability to manage diverse personalities with superior customer service skills.
14. Have a basic understanding of PDPM and Quality Measures.
15. Initiate and complete assessments, MDS, CAAS and care plans in accordance with the MDS process, on a timely basis.
16. Assures that all required assessments are complete, accurate and timely prior to and during the care conference.
17. Participate in discharge planning training, caregiver training as needed.
18. Participate in educating staff regarding documentation as needed.
19. Basic understanding of Managed Plans in relation to scheduled MDS’s and recertifications.
20. Understanding of NOMNC/ABN and appeals process for Traditional and Managed Plans.
21. Complies with ICMCF code of conduct, resident rights, infection control, policies and procedures, federal, state and professional requirements and regulations.
22. Maintains patient privacy and protects ICMCF MDS operations by keeping information confidential by following all HIPAA/confidentiality guidelines to assure rights are maintained and protected.
23. Participate in meetings as requested (Behavior, Discharge, Stand-up, QAPI, Falls, etc)
24. Understanding of Level of Cares and completes as required.
25. Supports the role of MDS Coordinator as needed.
26. Assume duties of RN charge nurse or staff nurse when required (see job description).
27. Performs other duties/responsibilities as directed by the Administrator, DON, MDS Coordinator, or their designee.
EDUCATION & TRAINING
1. Must possess a current, valid Michigan license as a Registered Nurse and have graduated from an accredited school of nursing.
2. Will consider an LPN with a valid Michigan license, have graduated from an accredited school of nursing, with two years of long-term care licensed nursing experience, as long as department already employs a RN backup MDS Coordinator.
3. Experience in MDS desired.
4. Knowledge of Nursing and Medical practices and procedures, as well as laws, regulations and guidelines that pertain to long term care setting
5. Minimum of two (2) years’ experience in a long-term care preferred

12. Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing practices.
13. Must have the ability to read and comprehend all written instruction and documentation associated with job. Must have ability to read, write and speak the English Language.
