Lead Case Management Director

A well-regarded facility in Petersburg, VA, is excited to present an opportunity for a Lead Case Management Director to join their team!

• Pay: $95,000 – $105,000 / annually

Lead Case Management Director Opportunity:

• Full-time, direct-hire position
• Schedule: Monday to Friday, day shift
• Open to both licensed registered nurses and social workers!

Lead Case Management Director Responsibilities:

• Oversee the daily operations and human resource management of the Case Management department
• Supervise the interdisciplinary care plan and discharge planning processes, focusing on patient care outcomes, census management, and key care indicators
• Provide leadership and support to Case Managers and staff, including training on case load management, regulations, policies, and operational procedures
• Foster relationships with insurance providers, employers, case management firms, and healthcare networks

Director of Case Management Qualifications:

• BSN or BSW degree required
• RN or social worker license is required for the Lead Case Management Director
• Minimum of 2 years of leadership experience in a hospital setting
• At least 3 years of hospital-based Case Management experience, including Utilization Review and Discharge Planning experience
