The Director of Case Management will oversee daytoday operations and human resource management of the Case Management department.Supervise the interdisciplinary plan of care and discharge planning process focusing on census management patient care outcomes and key care indicatorsProvide guidance and support to Case Managers and staff including training on caseload management regulations policies and operational proceduresBuild relationships with insurance companies employers case management firms and healthcare networksDirector of Case Management Qualifications :
BSN or BSWMust be either a Licensed Registered Nurse or a Licensed Social Worker2 years of leadership experience in a hospital setting3 of hospitalbased Case Management experience including Utilization Review and Discharge Planning experienceCurrent CCM or ACMTM certification is preferredThe Virginia Area :
