Director of Case Management Position : Salary : $85,000 $110,000 Schedule : Full-Time, Direct Hire, Day Shift Director of Case Management Responsibilities : Oversee the interdisciplinary plan of care and discharge planning process. Ensure services are effective and appropriate, with a focus on census management, patient care outcomes, and key care indicators. Lead and mentor the Case Management team to maintain high standards of care and compliance with regulations. Collaborate with clinical teams to address patient needs and improve care coordination. Monitor utilization review processes to ensure efficient resource use and compliance with payer requirements. Director of Case Management Qualifications : Experience : At least 3 years of hospital-based Case Management experience, including Utilization Review and Discharge Planning. Nursing Candidates : Must hold a bachelor s degree in nursing (BSN) with RN licensure. Licensure : SD State Licensure required. Other Professionals : Must hold a minimum of a bachelor s degree (graduate degree preferred).
