Exciting Full Time, Permanent Director of Case Management Opportunity near Houston, TX

Pay : $100,000 – $105,000 / Yearly

Quarterly Bonus!

Director of Case Management Responsibilities :

• Take charge of the daily operations and human resource management within the Case Management department.
• Supervise the interdisciplinary care plan and discharge planning processes, ensuring services are effective and appropriate, with a focus on census management, patient care outcomes, and key care metrics.
• Advocate for patients and their families, ensuring services align with their needs and expectations.
• Offer guidance and support to Case Managers and other team members, including training on caseload management, policy interpretation, regulatory compliance, and organizational procedures.
• Build and maintain strong relationships with insurance providers, self-insured employers, case management firms, and other healthcare networks.
