A homehealth hospice facility based out of Modesto CA is seeking an experienced Physical Therapist to join their team! If

Physical Therapist Salary : $125000 $145000 annually

Physical Therapist Opportunity :

Employment Type : Fulltime permanent
Schedule : Variable; seeing between 48 patients a day
Setting : Home Health
Assess and develop care plans : Conduct comprehensive evaluations of patients physical abilities and design individualized treatment plans in collaboration with the interdisciplinary care team
Implement interventions : Provide therapeutic exercises and mobility training to maintain or improve function comfort and quality of life for patients with terminal illnesses
Educate and support caregivers : Train family members or caregivers on proper body mechanics safe transfer techniques and ways to assist with daily activities to ensure patient comfort and safety at home

Physical Therapist Qualifications :

A Doctor of Physical Therapy (DPT) degree from an accredited program.

A license to practice Physical Therapy in California

At least 1 year of Physical therapy experience required

Experience in home health setting is a plus
