Ironwood Cancer & Research Centers has beautiful state-of-the-art integrated Cancer and Women's Centers, with locations strategically located throughout the Valley. Each one provides a superior cancer care environment with a full spectrum of cancer related services for patients. Our multi-disciplinary team approach includes surgical oncology, medical oncology, radiation oncology, women's centers, diagnostic imaging services, social service support, nutritionist, integrative services, and genetic counseling.

Schedule: Full-time, Hourly position, Monday through Friday day shift position. This position will be based out of our Central Business Office in Gilbert or Chandler.
Position Summary:
The Health Equity Coordinator is a specialized role dedicated to addressing and mitigating health disparities among diverse populations. This position is essential in developing and implementing strategies that ensure equitable access to healthcare services, thereby promoting fairness and justice in health outcomes across various demographic groups. The Health Equity Coordinator is committed to identifying and bridging gaps in health status and service access that affect marginalized communities. This role requires a comprehensive understanding of social determinants of health—factors such as socioeconomic status, education, neighborhood environments, and physical and mental health—that influence individual and group differences in health outcomes.
Essential Duties and Responsibilities:
- Employs critical thinking and judgment to collaborate and inform the care management process, facilitating appropriate healthcare outcomes for members through care coordination, support, and education.
- Utilizing care management tools and data review, the coordinator evaluates referred members' needs and provides resources to achieve case resolution.
- Identifies high-risk factors and service needs, referring patients to supportive care services such as dietitians, social workers, and financial counselors.
- Directs patients and families to available community resources and supportive services, connecting them with relevant community services like transportation, financial assistance, or support groups.
- Tracks patient progress and reports key metrics to relevant stakeholders.
- Assists in the development of patient education materials, policies and procedures, workflow pathways, and quality studies.
- Identifies and escalates quality of care issues through established channels.
- Completes tasks and orders with proper documentation on patient referrals and notifications.
- Manages administrative tasks, including faxing documents, sending and receiving patient messages via the patient portal, retrieving voicemails, and returning phone calls.
- Utilizes care management and quality management processes in compliance with company policies and procedures.
- This position requires the ability to travel in-state up to 10% of the time.
- Flexibility to work beyond the core business hours of Monday-Friday, 8am-5pm, is required.
Knowledge, Competencies & Skills:
- Effective communication skills with patients, healthcare providers, and community partners, both verbal and written.
- Awareness of cultural nuances and the ability to adapt communication styles accordingly.
- Ability to actively listen to patients' concerns and provide emotional support.
- Strong attention to detail.
- Proficient in computer software and database skills, with good knowledge of EMRs.
- Comfortable working in a constantly evolving environment.
- Proficient in asking probing questions to patients.
- Maintains HIPAA compliance.
- Efficiently manages patients, coordinates multiple tasks, and engages in proactive problem-solving.
- Ability to work in a fast-paced environment and meet deadlines.
Education and/or Experience:
- High school graduate or equivalent required; Associate degree preferred.
- A minimum of 2-5 years of healthcare experience, including medical terminology, is required.
