What are the responsibilities and job description for the Social Service Assistant position at Irvine Nursing and Rehabilitation Center?
POSITION PURPOSE :
Assist the Social Services Director / Coordinator in identifying and providing for each resident's social, emotional and psychological needs, and the continuing development of the resident's full potential.
ESSENTIAL FUNCTIONS OF POSITION :
- Assist in collecting social history data and in the development of a comprehensive discharge plan, including the MDS and Care Plan for new admits, as directed by the Social Services Director.
- Orient and assist new residents in adjusting to the facility. Assist residents and families with adjustment to changes, including room changes.
- Assist families with the engraving and / or marking of resident's personal items.
- Review resident's rights with residents and staff and attend to various personal needs of the residents.
- Document progress notes, which relate to each resident's care plan when necessary and within policy timeframe.
- Assist in facilitating family group meetings.
- Participates in departmental budget planning and supplies monthly report to Administrator.
- Notifies Supervisor if resident needs clothing.
- Perform additional duties as assigned.
EDUCATION / REQUIREMENTS :
Bachelor's Degree preferred plus a minimum of 1 year experience in social work, preferably in a long-term care facility; or equivalent combination of education and experience. Highest level of professionalism with the ability to maintain confidentiality. Ability to communicate at all levels of organization and work well within a team environment in support of company objectives.
