A Client Care Navigator (CCN) is a very important role within the Isaiah House organization. Navigators represent the treatment programs offered by Isaiah House to front-line referral agencies throughout the state of Kentucky.

A Client Care Navigator (CCN) is a very important role within the Isaiah House organization. Navigators represent the treatment programs offered by Isaiah House to front-line referral agencies throughout the state of Kentucky.
Job Specific Functions
- Build strong relationships with Centers of Influences (COI) in the CCN assigned Region
- Educate COIs on the history, programs and outcomes provided by Isaiah House
- Field Calls from potential clients who are exploring treatment options
- Manage travel schedule to counties specifically assigned
- Maintain database on all COI activity, Client referrals, Clients admitted in ASANA
- Communicate all new client referrals through CCN supervisor via MS Teams
QUALIFICATIONS:
- High school diploma or GED preferred
- Knowledge of addiction and substance abuse treatment
- Connections with community services, organizations, and other public groups is essential
- If in recovery, must be able to demonstrate one year of continuous sobriety
