Job Summary
iServe Healthcare hires a Private Duty Nurse, RN/LVN, for patient care in Paris, TX. Shift available M-F as well as part-time weekend coverage. The PDN Nurse is responsible for completing nursing assessments as required with timely and accurate documentation submission. The Nurse will consistently demonstrate commitment to the agency’s mission to provide quality services for all patients to allow them to remain in their homes and live with independence and dignity for as long as possible.
Roles and Responsibilities
1. Performs Pediatric Assessment Tools and comprehensive assessments to determine client eligibility.
2. Supports the Admission/Discharge Coordinator with tasks including but not limited to client admission assessments, services addition paperwork, and discharges.
3. Performs in-home and telemedicine visits as assigned, including supervisory visits, recertification assessments, and resumption assessments.
4. Documents visits and assessments thoroughly, accurately, and timely.
5. Acts as a role model for other iServe Healthcare employees and as a subject matter expert for clinical services staff.
6. Provides skilled nursing as required by the patient care plan.
8. Communicates all pertinent information to families, clinical management, and field staff effectively.
9. Protects confidential information and understands responsibilities regarding the Health Insurance Portability and Accountability Act (HIPAA) and protected health information (PHI).
10. Follows iServe Healthcare Infection Control Policy to include PPE as directed.
11. Pursues learning opportunities to enhance personal and professional capabilities relating to the assigned role.
12. Develops effective working relationships among professionals and ancillary staff.
Minimum Requirements
- Strong analytical and problem-solving skills to evaluate the quality of care services and to resolve issues.
- Texas RN or LPN license in good standing with the Department of Regulatory Agencies. Transfer to Texas is required within six months of hire if assigned in an alternate state.
- Ability to advocate for and relate to clients/families and their concerns.
- Current knowledge of laws and regulations related to home care.
- Ability to remain current in the skills and knowledge necessary to provide quality patient care.
- Strong computer skills. Ability to work with electronic medical records system, email, and Microsoft Office products
- Strong time management, organizational skills, and attention to detail.
- Ability to interact with clients and families compassionately and empathetically.
- Patience: Being patient with people who may have trouble communicating or understanding what is happening is essential.
- Communication Skills: Effective communication with patients, families, and staff must be adequate.
Physical/Mental Demands
While performing the duties of this job, the employee is regularly required to use their hands to handle objects, tools, or controls and communicate effectively with other people. The employee is occasionally required to stand; walk; sit; and reach with hands and arms. May be exposed to occupational hazards such as communicable diseases, harmful chemicals, and/or disoriented or combative patients. Lifting up to 50 lbs. may be required.
