Job Description

Position Summary: We are offerering a sign on bonus after cmpletion of three months employement. Supervise all aspects ofr the medication rrom and the medication staff, including scheduling, hiring, counseling and teminating when necessary. Properly assist with scheduled medications to residents in accordance to DOH rules and regulations, and facilities policy and procedure. Document in a timely fashion. Provide personalized care and services to residents. Ensure the residents rights to safe, prompt, and confidential treatment, at all times.

  • While assisting with medications, documenting or working to oversee the safety and well being of the residents in accordance with the facilities philosophy and standards of care.
  • Follow the "six rights" of medication assistance (Right; resident, drug, dosage, time, route, record) each and every time assistance with medication is provided. The "six rights' must be checked (a) when the medication is being prepared for assistance (removing it from the cabinet and placing in proper are (b) immediately prior to assisting resident with medication, (c) immediately after assisting with medication (at time you enter the information into the MAR).
  • Assist with medications according to MAR and record appropriately, making sure to uphold resident's privacy.
  • Document and file paperwork correctly and efficiently in a timely manner, case manage issues that require follow up.
  • Maintain a neat, clean, organized medication cart, medication and filing cabinets, and medication room.
  • Ensure that medication is ordered in a timely fashion and changes, hold, stop, increase, decrease, and new orders documented, and instructions from doctors and pharmacists are followed correctly and documented as such.
  • Care should be used to wash hands and keep a sterile environment in the medication room and when assisting with medications.
  • Establish and maintain an open door policy and a high level of ongoing communication with residents and their families.
  • Ensure that residents, their families, responsible parties and/or doctors are contacted when a change in service is needed.
  • Maintain confidentiality of resident's records.
  • If requested, explain to the resident what medication is being given and why.
  • Respect the resident's right to refuse medication assistance.
  • Promote and protect resident's rights to safe, prompt and confidential treatment.
  • Assist residents in meeting their physical, emotional, spiritual, and medical needs to achieve and maintain their highest level of wellness.
  • Provide quality care and assistance to residents in accordance with the facilities policy and procedure, and resident rights.
  • Observe the resident for any adverse reaction to a medication, report, and document it to your supervisor and/or the doctor immediately.
  • Control the spread of infection by following proper procedure.
  • Carry out all duties as assigned to you by your supervisor.
  • schedule and run regular and scheduled staff meetings and trainings.
  • Effectively communicate information pertaining to residents with appropriate staff members and the medication staff, according to facilities policies and procedures.
  • Effectively communicate to appropriate persons when necessary; doctors, family, resident representative(s), or administration, any changes in a resident's condition or behavior, adverse reactions to medication, any resident complaints, refusal of a medication, any errors noted on the MAR.
  • Notify your supervisor any concerns or questions presented by resident or their family members.
  • Responsible for all supervision, staffing and scheduling for the department.
  • Ensure a high level of customer satisfaction.
