Department: Health and Human Services (IMCare Division)Classification: Grade 11 (Local 580)FLSA Status: Non-Exempt; PR/PTPosition Description: Implements a care coordination program that focuses on improving the health status of program enrollees. The care coordinator program will focus on reducing health care costs through coordinating health and wellness services provided by contracted health care and human service providers. Care Coordinators provide direct and indirect enrollee interventions and support, ongoing assessments, and ongoing care plan management. Supports collection of enrollee and program specific information, evaluation of collected information, and presents quality improvement measures based on program and community needs. Provides teamwork approach to collaborate with peers and ensures enrollee needs are met in a timely manner. This job has the potential to work from home following a probationary period and depending upon the work produced during this period. This position does not include direct patient care and instead focuses on member education, continuity of care, and health maintenance.
