Job Summary
- The Case Manager role is part of the Behavioral Health Team that supports ETS’ mission through outreach, engagement, and intensive case management services.
- This role makes a critical difference at ETS by providing direct support to individuals experiencing homelessness, substance use, and co-occurring disorders. Services include care coordination, community resource linkage, and addressing individual needs to promote stability and improve health outcomes. The Case Manager plays a critical role in fostering relationships with community partners, ensuring continuity of care, and advocating for systemic change to address gaps in services.
- Help ETS succeed in carrying out our mission through working together with other staff to transform systems of harm and inequity to create different approaches to improving community health and safety through addressing substance use and homelessness.
- Assist with the organization-wide initiative to reimagine our interconnectedness within our community to overcome the aspects of our society and organizational culture shaped by white supremacy and settler colonialism.
- Participant Engagement & Welcome: Serve as the first point of contact for individuals entering the organization. Ensure they feel welcome and provide them with an overview of available services.
- Comprehensive Intake Assessments: Collect and document personal information, service history, and immediate needs. Perform initial screenings for eligibility and suitability for internal programs.
- Service Plan Development: Work with individuals to create initial care plans, connecting them to appropriate services based on their immediate needs and long-term goals.
- Consent & Documentation: Ensure all required consent forms and documentation are signed and filed according to organizational policy and regulatory requirements.
- Appointment Scheduling: Coordinate appointments for individuals with internal services, including case management, counseling, medical appointments, and other necessary resources.
- Referral & Follow-up: Assist internal service providers with making referrals to outside agencies and help participants navigate barriers to attending referral appointments.
- Service Navigation: Act as a liaison between individuals and service providers within the organization, ensuring timely access to services and addressing any logistical challenges e.g., transportation, scheduling conflicts.
- Barrier Identification: Identify potential obstacles to service engagement, such as financial constraints, childcare, or transportation, and work with internal teams to find solutions.
- Collaboration with Providers: Facilitate warm hand-offs to relevant team members (case managers, counselors, medical professionals) for more specialized services, ensuring smooth communication and a cohesive care approach.
- Build rapport and develop trusting relationships with participants through outreach activities, including meeting participants in the community, at shelters, healthcare facilities, and on the streets.
- Conduct needs assessments to identify barriers to housing, healthcare, and stability, and work collaboratively with participants to address those barriers.
- Provide ongoing engagement and support to individuals who are not yet ready to participate in services, utilizing harm reduction and trauma-informed approaches.
- Collaborate with participants to create and regularly update Individualized Service Plans (ISP) that address their needs, goals, and priorities, such as housing, medical care, mental health support, substance use disorder treatment, employment, and other identified areas of need.
- Review and adjust service plans based on participant progress, emerging needs, and changes in life circumstances.
- Encourage and support participants in their journey toward increased independence and self-sufficiency, using motivational interviewing techniques and participant-driven goal setting.
- Assist participants with locating, applying for, and when available securing stable housing.
- Provide ongoing case management to help participants maintain housing stability by addressing barriers such as income, legal issues, or behavioral health challenges.
- Act as a liaison between participants and landlords, advocating on behalf of participants to resolve disputes or issues related to tenancy.
- Medical and Behavioral Health Coordination:
- Assist participants in accessing primary medical care and managing chronic health conditions by coordinating appointments, transportation, and follow-up care.
- Link participants with mental health services, substance use treatment programs, and other behavioral health services, ensuring they receive the necessary support for recovery.
- Monitor participants' adherence to treatment plans and work with other providers to adjust interventions as needed to meet changing circumstances.
- Utilize de-escalation techniques and coordinate with emergency services when necessary to ensure participant safety and well-being.
- Work collaboratively with participants during crises to develop safety plans and provide follow-up support.
- Link participants to needed services for crisis intervention and support.
- Assist participants in applying for and maintaining access to state/federal programs such as Social Security, disability benefits, food assistance, and health insurance.
- Support participants in accessing education and vocational programs, including job readiness training, resume building, and interview preparation.
- Collaborate with workforce development programs to help participants secure and sustain employment, offering guidance and advocacy where needed.
- Advocacy and Systems Navigation:
- Advocate for participants' rights and needs within the healthcare, housing, legal, and social service systems.
- Help participants navigate complex bureaucratic systems to access resources such as public benefits, legal aid, and transportation assistance.
- Identify and address gaps in community resources by advocating for systemic change, attending coalition meetings, and participating in task forces as necessary.
- Maintain accurate, up-to-date documentation of all participant interactions, services provided, and progress towards goals in compliance with agency, state, and federal guidelines (including HIPAA and 42 CFR, Part 2).
- Prepare reports on participant outcomes, program successes, and barriers encountered, contributing to continuous quality improvement within the program.
- Participate in regular case reviews and ensure that all records meet auditing standards.
- Work closely with other case managers, healthcare providers, housing specialists, and community partners to coordinate services and ensure continuity of care for participants.
- Actively participate in multidisciplinary team meetings, case conferences, and staff trainings to share insights, develop intervention strategies, and support professional development.
- Assist with program development and provide feedback for ongoing improvement of case management practices and participant outcomes.
- Maintain professional boundaries with participants and colleagues, ensuring relationships are supportive and respectful.
- Protect participant confidentiality at all times and adhere to all legal and ethical standards, including HIPAA regulations and confidentiality provisions under 42 CFR, Part 2.

What you bring to this role – Qualifications:
- Education and/or Relevant and Lived Experience (if applicable):
- High School Diploma required; associate or bachelor's degree in social work, human services, psychology, or related field preferred.
- At least two years of experience working with individuals experiencing homelessness, substance use, or behavioral health challenges. Experience in harm reduction and outreach programs is highly desirable.
- Have an understanding of racial justice and social equity and a commitment to helping create an equitable environment for all ETS clients and patients as well as fellow staff.
- Ability to establish and maintain effective working relationships with clients, patients, and staff from a wide variety of ethnic, socioeconomic, and cultural backgrounds.
- Strong interpersonal skills and verbal/ written communication skills.
- Excellent organizational skills and ability to prioritize workload, work independently, and complete tasks timely and efficiently.

Physical Conditions and Requirements:
- The employee may be exposed to illicit drug residues and fumes or other bio-hazardous materials when carrying out job functions. There is also potential for exposure to bloodborne pathogens. ETS will provide employees with appropriate training to limit the risk of exposure to bloodborne pathogens. Policies and procedures are in place addressing each item specifically.
- The employee is regularly required to sit; use hands to finger, handle or feel objects, tools, or controls; reach with hands and arms and talk or hear; frequently required to stand, walk, and kneel; occasionally to climb balance, or stoop; rarely to crouch or crawl.
- The employee must occasionally lift and/or move up to 30 pounds. Specific vision abilities required by this job include close, color, and peripheral vision and the ability to adjust focus. The noise level in the work environment is moderate.
