Home Infusion Pharmacist

IV Care Options has an opportunity for an experienced pharmacist. Duties include: reviewing prescriptions issued by physicians to assure accuracy and determine formulas and ingredients for multi-dose IV administration, infusion related drug preparation, dispensing of prescribed infused medications, knowledge of IV nutritional products and special medications (IVIG, pain management). Directs pharmacy techs in mixing, packaging and labeling pharmaceuticals. Enters patient data, medications and inventory into database.
Minimum Requirements
Graduate of an accredited School of Pharmacy
Licensed and in good standing with the State Board of Pharmacy in state of practice
Self-directed, able to work without the presence of an immediate supervisor
Excellent communication skills, flexibility and ability to work with all types of people
Current valid driver’s license
Proof of current automobile insurance
Preferred Qualifications
Experience with or knowledge of admixture procedures, 2 years hospital or 1 year home care practice
Supervisory experience
Post-graduate residency program in clinical pharmacy services
