The Clinical Infusion RN Liaison will be a front-line clinical contact, acting as a direct extension of IV Care Infusion Pharmacy. The liaison instructs, motivates, and assists patients and caregivers in learning and improving the ability to independently administer infusion therapies in the home. The liaison may also be asked to perform alternate setting visits to provide infusion nursing interventions as ordered by the physician.
ESSENTIAL FUNCTIONS:
Identify Service needs for patients, caregivers, and referral sources. Demonstrates a working knowledge of the referral process, insurance coverage, necessary documentation, and therapies provided.
Ability to connect with patients to educate them in aseptic technique and administration of IV therapy during a hospital liaison visit.
Intake home infusion orders from hospital and alternate sites, ensuring the collection of accurate and complete information.
Provide the training required for a seamless transition from hospital to home.
Effectively communicates with referral sources, physicians and family to coordinate the plan of care, acting as a liaison between the patient and other members of the Health Care Team
Collaborates with intake, customer service, and clinical team to help improve efficiency in processing new patient orders.

Education and Experience Requirements:
Graduate of an accredited School of Nursing and unrestricted licensure as a Registered Nurse in the states of Missouri and willing to become licensed in Illinois.
2 years of Home Infusion or Hospital experience preferred.
Current CPR Certification preferred.
Must have a valid and unrestricted driver's license.
Physical Demands:
Work environment may be stressful at times.
May be exposed to hazardous materials, bloodborne, airborne or other pathogens.
Must be able to travel with the use of a personal vehicle.

Pay: $41.00 - $46.00 per hour
